The Patient, Family and Caregiver (PFAC) Council serves as a platform for the voices of patients, clients, family members, caregivers, and community members of the North Simcoe Ontario Health Team. A patient, family, caregiver, and community member is a person who brings experiences, along with the inclusion of all perspectives – especially the vulnerable populations – of what it is like to be a patient, client, family member or caregiver. The goal of this group is to collaborate on working group projects and decision making for a common cause – to improve the way people access and receive health care in North Simcoe.

The North Simcoe Ontario Health Team is actively seeking patients, family, caregivers, and community members to join the PFAC Council. This group is for those who are interested in sharing their experiences with health and social services, and to inform and improve the way people access and receive care.
